It is important that we have accurate and up-to-date information about your medical and dental history so that we can look after you safely. Please complete New Patient Medical History Form prior to your visit if you are a new patient.

If you are an existing patient, we update your medical and dental history information annually using this form.

If you have records from a previous dental practice including x-rays, we are able to conveniently request them on your behalf after you complete this form to comply with Privacy Laws.

New Patients

New Patient Comprehensive Examination

At Kingston Beach Dental our New Patient Comprehensive examination is a 1 hour appointment.   We obtain a complete and accurate picture of your oral health needs and will discuss and explain your treatment options with you, in language that you understand. Please complete this medical history form prior to your appointment.

Our thorough and comprehensive package includes:

  • Examination including checking and charting all teeth to assess the function of your bite; to diagnose decay and any other dental issues and required treatment.

  • OPG x-ray which provides a detailed image from ear to ear and nose to chin to get baseline information about the present condition of your teeth, jaw and bone.   This allows detection of impacted wisdom teeth, infections, cysts, and early diagnosis of tumours or bone cancer.  It is taken every 5 years.

  • Bitewing x-rays are taken every 2 years to check for periodontal disease and to detect decay between the teeth or underneath fillings which are not visible with the eye.

  • Photographic records to record the condition, shape and shade of all your teeth and the soft tissues to monitor and record the improvements in your oral health and to plan, explain and document your treatment.

  • Oral cancer screening to check for abnormalities.

  • Assessment of the gums and soft tissues (lips, gums, tongue, and palate) including diagnosis of gum disease or periodontitis.

  • A scale and clean will usually be performed at this visit, after we have assessed the health of your gums.  If you have gum disease or periodontitis you will require a deeper clean over a couple of visits, or you may need us to refer you to a specialist periodontist.  If it has been a long time since your last dental visit we may also perform your clean over several visits for your comfort.

Once you are a patient of our practice we encourage you to attend every 6 months to ensure that small problems don’t become bigger and more expensive to fix. Regular attendance improves your oral health and has a significant impact on your overall health.
